This notice explains what this site collects when you complete the orientation, self-assessment and attestation, how we use it, and who can see it. We wrote it to be read. If anything is unclear, write to us at peeracademy@formcommunities.org.
Who runs this site
This site is operated by Form Communities, the Texas organization behind Peer Academy and PeerHubTX. It hosts the orientation and self-assessment that the Texas Health and Human Services Commission (HHSC) asks prospective peer specialists to complete, and it produces your Attestation of Completion. It is not an HHSC website.
What we collect
Your attestation: the name you type as your signature, the date, the exact wording of the statement you attested to, and which readiness statements you checked. The reflection questions are for you alone. We record only that they were shown, never any answers.
Your email address, so we can send you your attestation.
Your county, so we can understand where in Texas interest in peer specialist training is coming from.
Technical information every website receives, such as your IP address and browser type. We use it only to keep the site working and to block automated abuse.
How we use it
To create your Attestation of Completion, email you a copy, and keep a copy so you can ask for it again.
To understand interest in peer specialist training across Texas. For that we look at counts by county, not at individuals.
If you later create an account with a training entity that uses the same platform, we may offer to attach this attestation to your application so you don’t have to complete this step twice.
We do not use your information for advertising, and we will not send you marketing email.
Who can see it
Form Communities staff who administer this site.
HHSC and Texas peer-workforce partners may receive de-identified counts, such as how many people in each county completed the orientation. They do not receive your name or email from us unless you ask us to share it.
The secure, access-controlled cloud services that host the site and deliver email.
We never sell personal information.
How long we keep it
We keep your attestation record and contact details so you can request a copy and so it can be honored if you apply for training. You can ask us to delete your information at any time. If you do, we will also delete our copy of your attestation, so save yours first.
Your choices
You can request a copy of what we hold about you, ask us to correct it, or ask us to delete it by emailing peeracademy@formcommunities.org. A real person will respond.
Cookies
This site sets no advertising or analytics cookies. A bot-protection check may set a short-lived technical cookie so the form can tell people from automated scripts.
Children
Texas requires peer specialists to be at least 18. This site is for adults, and we do not knowingly collect information from anyone under 18.
Changes to this notice
When this notice changes, we update the date above. Your attestation record notes the version of this notice that was in effect when you signed.
